About the Role

We are seeking an experienced Mental Health Practitioner to join our established and supportive Older People's Community Mental Health team at Norfolk & Suffolk Foundation NHS Trust. As a key member of our multidisciplinary team, you will work collaboratively with colleagues to provide high-quality care to older adults with complex mental health needs.

Key Responsibilities
  • Manage a caseload of patients with organic and functional presentations under CPA and NCPA
  • Assess and manage risk
  • Use Dialog+ and Recovery Care Planning with service users and carers
  • Communicate effectively through notes on Lorenzo and letters to Service Users, GPs, and other involved parties
  • Administer depot medication
  • Drive independently for work-related purposes
