Care Manager for Hospital Discharge

We are looking for a skilled professional to join our team at Charles Hunter Associates as a Care Manager for Hospital Discharge. This role requires strong social work qualifications and experience.
About the Team:
Our team works in a fast-paced environment, creating care plans for effective hospital discharges. The ideal candidate will have a degree in social work or a related field and a minimum of 1 year's post-ASYE experience. Previous experience working within safeguarding hubs, hospitals, communities, and with older people and physical disabilities is essential.
Salary: £45,000 - £55,000 per annum (dependent on experience)
Key Responsibilities:

- Use strength-based approaches to identify creative solutions
