Short-Term Assessment and Reablement Team

Key Responsibilities:

  • Work with individuals in their homes or homely settings to support short-term reablement programs.
  • Contribute to a multi-disciplinary team, including Reablement Workers, Care and Support Supervisors, Occupational Therapists, and Physiotherapists.
  • Support individuals in regaining mobility, strength, and independence.

Requirements:

  • Essential: Clean UK driving license and access to a vehicle.
  • Desirable: Willingness to travel.
