Job Summary:

We are seeking a skilled and dedicated Community Staff Nurse to join our team in the Westside community Nursing Team. The successful candidate will provide high-quality nursing care to patients in their own homes, working effectively as a team member to deliver excellent patient outcomes.

Key Responsibilities:

  • Provide nursing care to patients in their own homes, working in collaboration with other healthcare professionals.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
  • Work effectively as a team member, contributing to the ongoing work around integration and supporting the Scottish government's 20 20 – 20 30 vision.
  • Participate in working a TW shift and a weekend service on a rota basis, demonstrating a flexible working approach.

Requirements:

  • Registered Nurse with a valid UK driving license and a car.
  • Experience of working within a community nurse setting, with a strong understanding of the principles of community nursing.
  • Ability to work effectively in a team environment, with excellent communication and interpersonal skills.
  • Flexibility to work in other areas of the Western Isles to provide nursing cover as required.
