About the Role

We are seeking an experienced and professionally registered Care Coordinator to join our Specialist Community Perinatal Team. The successful candidate will have a strong background in working with women experiencing perinatal mental health needs and a clear understanding of the theory and practice of Recovery Principles.

Key Responsibilities
  • Undertake comprehensive mental health assessments for service users with severe and enduring mental health needs.
  • Develop and deliver complex treatment programmes using evidence-based practice.
  • Act as a care coordinator for service users with the most complex needs.
  • Provide defined interventions to individuals on other workers' caseloads.
  • Deliver a range of defined activities/interventions to improve carers' ability to support service users.
  • Develop and maintain good partnership working with other services throughout treatment episodes.
  • Facilitate the development of complex comprehensive crisis plans, rapid access plans, and advance statements.
  • Build and lead hope-inspiring relationships with service users, acknowledging their personal journey and focusing on strengths and aspirations.
  • Maintain own workload and plan the workload of others, ensuring effective time prioritization.
  • Protect individuals from abuse and harm in line with local safeguarding policies and procedures.
  • Undertake supervision for other colleagues within the team and cover for the manager and colleagues as needed.
Person Specification
  • Registered as an OT, SW, or RMN (parts 3 or 13 of NMC register).
  • Formal post-graduate diploma level qualification in a relevant field.
  • Significant post-qualifying clinical experience obtained in a variety of settings.
  • Experience of working with women experiencing perinatal mental health needs/knowledge of demographic risk factors for this client group.
  • Clear record of ongoing continuing professional development.
  • Demonstrates experience as a Care Coordinator or equivalent role of complex cases.
  • Demonstrates a detailed understanding in the theory and practice of Recovery Principles.
