About the RoleWe are seeking a highly skilled and experienced Care Coordinator to join our Specialist Community Perinatal Mental Health Service team. The successful candidate will be responsible for providing assessment and care coordination for an allocated caseload of service users with perinatal mental health needs.
Main Duties and Responsibilities- Undertake comprehensive mental health assessments for service users with severe and enduring mental health needs.
- Develop, deliver, and review complex treatment programmes using evidence-based practice.
- Act as a care coordinator for service users with the most complex needs, providing defined interventions and support.
- Deliver a range of defined activities/interventions to improve carers' ability to support service users.
- Develop and maintain good partnership working with other services throughout treatment episodes.
- Facilitate the development of complex comprehensive crisis plans, rapid access plans, and advance statements.
- Build and lead hope-inspiring relationships with service users, acknowledging their personal journey and focusing on strengths and aspirations.
- Maintain own workload and plan the workload of others, ensuring effective time management and prioritization.
- Protect individuals from abuse and harm, working with others to plan, develop, and review protection plans.
- Undertake supervision for other colleagues within the team, covering for the manager and colleagues as needed.
- Maintain the single health and social care record, ensuring both paper and electronic records are up to date.
- Report and record activity relating to information reporting and performance within agreed timeframes.
- Participate in management, caseload, and clinical supervision in accordance with trust policies.
- Provide mentoring/training for others in relevant practice areas, taking a collaborative approach to practice development and evidence-based care.
Education and Qualification
- Registered as an OT, SW, or RMN (parts 3 or 13 of NMC register)
- Formal post-graduate diploma level qualification in a relevant field
Experience and Knowledge
- Significant post-qualifying clinical experience obtained in a variety of settings
- Experience of working with women experiencing perinatal mental health needs/knowledge of demographic risk factors for this client group
- Clear record of ongoing continuing professional development
- Demonstrates experience as a Care Coordinator or equivalent role of complex cases
- Demonstrates a detailed understanding in the theory and practice of Recovery Principles
Desirable
- Experience or delivery of training/teaching within MDT/external organisations
Skills and Abilities
- Able to articulate an advanced and detailed understanding of the relevant legal Frameworks
- Demonstrates a clear knowledge and understanding of the issues and statutory processes involved in caring for vulnerable people
- Supervisory experience skills including clinical supervision, appraisals, etc
