About the Job

We are seeking an experienced Reablement Care Specialist to join our team at Norfolk Community Health and Care NHS Trust. In this role, you will be responsible for delivering and coordinating care to a named group of patients, promoting reablement care and safe control of drugs within the ward/department.

Job Summary
  • Provide high-quality patient care, developing and delivering care plans designed to meet individual needs.
  • Work collaboratively as part of a multidisciplinary team, promoting effective teamwork and communication.
  • Contribute to the development of the team, utilising your skills and experience to drive improvement and innovation.
Key Responsibilities
  1. Develop and implement care plans, taking into account patient preferences and needs.
  2. Monitor and evaluate the effectiveness of care plans, making adjustments as necessary.
  3. Collaborate with other healthcare professionals, sharing knowledge and best practices to enhance patient care.

The ideal candidate will possess excellent communication skills, along with a strong understanding of reablement care principles and current clinical/professional issues. Experience of working in a multidisciplinary team environment is also essential.
