We are seeking a highly motivated and experienced Care Coordinator to join our team at Essex Partnership University NHS Foundation Trust. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals experiencing first episode psychosis.
Key Responsibilities- Undertake care coordination of a caseload of clients, ensuring effective management of complex needs.
- Conduct initial health and social needs assessments, and participate in multi-disciplinary team meetings to allocate clients to care plans.
- Develop, implement, and evaluate recovery-based care plans, working collaboratively with clients, carers, and other healthcare professionals.
- Prioritize and manage workload to ensure effective management of a defined group of clients with complex needs.
- Participate in annual audits relevant to the clinical pathway and contribute to the improvement of services provided to clients/patients.
- Appropriate professional registration, such as RMN or equivalent.
- Experience of working with individuals experiencing first episode psychosis, including assessment, care planning, and implementation.
- Strong communication and interpersonal skills, with the ability to work effectively with clients, carers, and other healthcare professionals.
- Ability to prioritize and manage workload, ensuring effective management of a defined group of clients with complex needs.
