Job Summary

We are seeking an experienced Care Coordinator Specialist to join our team in providing comprehensive assessment, diagnostic, and treatment pathways for patients with dementia and those experiencing age-related frailty.

Main Responsibilities:
  • Assess, plan, implement, and evaluate packages of care based on individual service user needs, under the guidance of the Advanced Nurse Practitioner.
  • Supervise and manage junior team members, ensuring adherence to Trust policies and procedures.
  • Collaborate with multidisciplinary teams, including clinicians, practitioners, and partner organizations, to deliver services 24/7, 365 days a year.
Requirements:
  • RGN or RMN qualification with experience in performance management and supervision.
  • Understanding of health legislation, principles of CPA, and risk assessment/management.
  • Excellent communication and interpersonal skills.
  • Current driving license and ability to travel across sites and trust boundaries.
Benefits

We offer a competitive salary range of £43,041 - £57,259 per annum, depending on experience. Additional benefits include season ticket loans, NHS discounts, excellent training facilities, buying and selling annual leave scheme, and salary sacrifice schemes.
