About the Role

We are seeking a highly skilled and compassionate Substance Misuse Nurse to join our team at Change Grow Live. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals struggling with substance misuse.

Key Responsibilities
  • Undertake comprehensive physical and mental health assessments for individuals accessing our services, ensuring timely and effective signposting to primary or secondary healthcare services as needed.
  • Deliver alcohol assessments, utilizing the outcome to determine safe detoxification and provide education, health promotion, and screening for blood-borne viruses (BBV).
  • Work within and consolidate integrated care and treatment pathways for individuals identified as being infected with BBVs, ensuring adherence to best practice guidelines and protocols.
  • Provide clinical leadership within the service, including coaching, teaching, and supporting staff in harm reduction and health promotion interventions.
  • Contribute to the overall performance of the service, ensuring contractual output targets are achieved and collaborating with colleagues to design and deliver community opioid and alcohol detoxification plans.
Requirements
  • A clear understanding of harm reduction and health promotion interventions, with the ability to assess and co-produce recovery plans with individuals accessing our services.
  • Sound knowledge of clinical prescribing issues, with experience of contributing to project development and knowledge of local services and geography.
  • First-level registered nurse with a current PIN, and a commitment to safeguarding best practice and treating all information acquired through the course of employment as confidential.
