Care Coordinator

About the Role
The Lache Health Centre is seeking a skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing and coordinating personalised care to our patients, working closely with our multidisciplinary team to promote and embed the personalised care approach across the Primary Care Network.
Main Responsibilities
- Work closely with clinical and admin teams to provide and coordinate care to patients
- Provide patient contact, including face-to-face and telephone interactions
- Administrate tasks and duties, including patient recall for health checks, QOF, and vaccinations
- Support the team with social media and EMIS reporting
- Provide reception desk support and batch messages via ACCURX
Person Specification
- Experience of working within healthcare or the voluntary/community sector, supporting vulnerable groups
- Experience of working within multi-professional team environments
- Experience of supporting people, their families, or their carers in a related role
- Strong organisational skills, including planning, prioritising, time management, and record keeping
- Good IT skills, including Microsoft Office applications and the ability to learn new software packages
- Ability to actively listen, empathise, and provide personalised support in a non-judgemental way
- Ability to provide a culturally sensitive service, supporting people from all backgrounds and communities
