Active Care Group's Care in the Home division is a leading provider of home care services, and we are seeking a highly skilled Care Coordinator to join our team.

We are committed to delivering high-quality, person-centered care to our clients, and our Care Coordinators play a vital role in ensuring that our clients receive the best possible care in the comfort of their own homes.

As a Care Coordinator, you will be responsible for coordinating and scheduling our care packages, providing a premium customer service, and working closely with our care teams to ensure that our clients receive continuity of care.

Our care in the home services support children, young people, and adults with a wide range of conditions, including brain injury, spinal cord injury, neurological conditions, and neurodegenerative diseases.

Key Responsibilities:

  • Coordinating and scheduling care packages
  • Providing a premium customer service
  • Working closely with care teams to ensure continuity of care
  • Managing on-call rotas and ensuring that our clients receive 24/7 support
  • Ensuring compliance with company policies and procedures

Requirements:

  • Experience as a Care Coordinator
  • Previous experience of using booking and phone systems
  • Ability to deal with conflict in a calm and supportive manner
  • Strong work ethic and a dynamic and flexible approach to the role
  • Excellent customer service skills
  • Ability to work as a team and think outside the box
