**Job Summary**

We are seeking a dedicated and compassionate Clinical Social Worker to join our team at Care New England Health System. As a key member of our healthcare team, you will provide essential social work services to patients and their families, addressing complex psychosocial and environmental factors impacting their health and well-being.

**Responsibilities**

  • Provide high-quality clinical care to patients and their families, engaging with the interdisciplinary team to address complex psychosocial and environmental factors.
  • Develop and implement comprehensive care plans, collaborating with the healthcare team to ensure seamless transitions and optimal patient outcomes.
  • Maintain accurate and detailed documentation, including clinical assessments and follow-up notes in patient medical records.

**Requirements**

  • A Master's degree in Social Work or related field.
  • Certification as a Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC).
  • Licensure as an Independent Clinical Social Worker (LICSW) strongly preferred.

**Estimated Salary:** $95,000 - $125,000 per year, depending on experience and qualifications.
