We are seeking a highly skilled Social Work Specialist to join our team at Care New England Health System. As a Social Work Specialist, you will provide comprehensive social work services to patients and families in our hospital setting.

Responsibilities:

  • Assess patient needs and develop effective discharge plans
  • Coordinate services and communicate with interdisciplinary teams
  • Provide education and support to patients and families

Requirements:

  • A Master's degree in Social Work from an accredited program
  • A license as a Clinical Social Worker (CSW) or Licensed Mental Health Worker (LMHW)
  • At least 2 years of experience in a healthcare setting, preferably in a hospital environment

Salary and Benefits:

  • An estimated salary range of $70,000 - $90,000 per year
