As a key member of our multidisciplinary team, you will play a crucial role in empowering recovery and safeguarding vulnerable young people. Your primary responsibilities will include coordinating and processing aftercare plans, assisting with safeguarding referrals, and conducting thorough social care assessments.
Main Responsibilities:
- Develop and maintain relationships with community agencies to ensure effective aftercare planning.
- Liaise with Local Authority safeguarding teams and LADO to ensure seamless referral processes.
- Assess and address the social care needs of young people in a low secure service.
- Implement care work and facilitate group sessions as needed.

- A competitive salary of £32,000 - £42,000 per annum, depending on experience.
