Key Responsibilities

  • Provide personalized care and support to customers in their own homes, including assistance with daily tasks, medication management, and social activities.
  • Develop and maintain strong relationships with customers, their families, and other healthcare professionals to ensure seamless care coordination.
  • Work collaboratively with the care team to identify customer needs and develop tailored care plans.
  • Conduct regular assessments and reviews to ensure customer needs are met and care plans are updated accordingly.
  • Participate in ongoing training and professional development to enhance skills and knowledge.

Requirements

  • Valid driving license and access to a reliable vehicle.
  • Ability to work flexibly, including evenings, weekends, and public holidays.
  • Excellent communication and interpersonal skills.
