About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team in Primary Care. As a key member of our community services, you will play a vital role in delivering high-quality mental health care to our patients.

Key Responsibilities
  • Contribute to the development and delivery of our primary care mental health service model, working closely with our community-based services and specialist teams.
  • Provide expert assessment and treatment to patients with mental health needs, working collaboratively with our primary care teams and other community services.
  • Build strong relationships with our patients, carers, and families, providing support and guidance to help them navigate the mental health system.
  • Work closely with our social prescribers and recovery navigators to ensure seamless transitions between services and provide a comprehensive approach to care.
  • Develop and deliver training and education programs for our primary care teams, promoting best practice and improving mental health outcomes.
  • Contribute to the development of our community mental health services, working collaboratively with our specialist teams and other stakeholders to improve patient outcomes.
Requirements
  • Professional qualification in mental health, such as a Registered Mental Health Nurse, Registered Social Worker, or Registered Therapist.
  • Current registration with a professional body and commitment to continuing professional development.
  • Significant experience of working in community mental health services, with a strong understanding of the needs and challenges of our patients.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and colleagues.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
