About the Role:

We are seeking an experienced and skilled Nurse Advisor Continence to join our team at Xcellin. As a key member of our healthcare team, you will play a vital role in providing expert care and support to patients with continence issues.

Key Responsibilities:

  • Provide comprehensive continence care to patients, including assessment, diagnosis, and treatment planning.
  • Work closely with multidisciplinary teams to ensure seamless care delivery and coordination.
  • Develop and implement individualized care plans to meet the unique needs of patients.
  • Collaborate with healthcare professionals to ensure continuity of care and optimal patient outcomes.
  • Conduct regular reviews and assessments to monitor patient progress and adjust care plans as needed.

Requirements:

  • Registered Nurse with a valid nursing license and experience in continence care.
  • Strong clinical skills and knowledge of continence care principles and practices.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Proficiency in electronic medical records and other relevant software systems.
