Job Overview
We are seeking a skilled and motivated individual to join our Community Rehabilitation Team as a Mental Health Practitioner. The successful candidate will work as part of a dynamic multi-disciplinary team, providing rehabilitation services to individuals with complex care needs.
The appointed candidate will assist care co-ordinators in delivering a recovery-focused service, working within NHS rehabilitation inpatient units, out-of-area placements, and the community.
The ability to communicate effectively with service users, families, and other healthcare professionals is essential. A commitment to developing knowledge and skills is also required.
Main Duties
Key responsibilities include:
- Assisting care co-ordinators in delivering a recovery-focused service
- Working with service users, families, and other healthcare professionals to achieve positive outcomes
- Developing and maintaining effective communication skills
- Participating in team meetings and contributing to the development of the Community Rehabilitation Team
