Job Overview

To provide high-quality care and support to individuals with mental health needs, working in partnership with multidisciplinary teams to achieve optimal health outcomes.

Main Responsibilities

  • Deliver frontline care and support to elderly and vulnerable individuals with mental health problems, including dementia and depression.
  • Enable individuals to maintain independence and participate in local community activities, promoting good mental and physical wellbeing.
  • Work collaboratively with patients, relatives, and professionals from partner organisations.

Requirements

The ability to speak Welsh is desirable, and we welcome applications from Welsh and/or English speakers.
