Care Coordinator

In this role, you will be responsible for providing support to individuals with intellectual disabilities and autism. Your primary focus will be on ensuring their safety, well-being, and independence.
You will work as a part-time Direct Support Professional (DSP) in our Life Sharing program teams. Our goal is to help individuals maintain or increase their independence and live their best lives.
Key Responsibilities:
- Implement individual plans of care, prioritizing high-quality support.
- Maintain accurate and timely documentation of daily activities.
- Communicate effectively with Coordinators regarding individual needs and progress.
Work Schedule:
- Up to 13 hours per week.
- Flexible scheduling to accommodate individual needs.
Requirements:
- One year of direct care experience working with individuals with intellectual disabilities.
- A high school diploma or equivalent.
Driving Requirements:
- A valid driver's license.
- Access to a reliable vehicle during work hours.
Estimated Salary: $17/hour, adjusted based on education, experience, and cases assigned.
