Job Overview
Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust is seeking a skilled Community Mental Health Specialist to join our team. This role involves working collaboratively with patients to develop care plans, recovery, and wellbeing plans.

About the Role
We are looking for a dedicated Care Coordinator Practitioner to work in our positive behaviour support pathway. The successful candidate will deliver high-quality assessment and care while offering support, advice, and supervision to the wider team.

Responsibilities
* Deliver high-quality assessment and care services to patients* Develop and implement individualised care plans, ensuring regular reviews to monitor progress and adapt care plans as necessary* Work collaboratively with patients to promote recovery, wellbeing, and maximise independence* Provide clinical supervision to staff as delegated by the Pathway Manager and Clinical Leads

Requirements
* 12-24 months of experience as a qualified nurse* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection* Experience of working in a community team* Able to maintain professional development

Estimated Salary: £35,000 - £45,000 per annum
