**Job Summary**
CNTW NHS Foundation Trust is seeking an experienced Specialist Mental Health Practitioner to join our dynamic team. As a key member of our multidisciplinary team, you will play a vital role in providing specialist care co-ordination and treatment for individuals with complex mental health needs.
The ideal candidate will possess strong assessment and risk management skills, with the ability to provide comprehensive written assessments and safety plans. You will be passionate about delivering high-quality care and have a proven track record of working with vulnerable populations.
In this role, you will work closely with various stakeholders, including community teams, inpatient staff, and third sector providers. You will be responsible for adapting trauma stabilization and structured clinical management approaches to meet the unique needs of each service user.
**Key Responsibilities**
- Provide specialist care co-ordination and treatment for individuals with emotionally unstable personality disorder diagnosis, co-existing conditions, and high risk.
- Develop and implement individualized care plans, incorporating evidence-based clinical approaches.
- Collaborate with multidisciplinary teams to ensure seamless care transitions and effective communication.
- Conduct regular risk assessments and develop safety plans to mitigate potential risks.
**Requirements**
- Registered Nurse (RNMH, RMN, RNLD) or equivalent AHP, Social Worker, or Clinical Psychologist qualification.
- Post-registration training in clinical/psychological interventions/skills relevant to the post.
- Proven track record of post-registration clinical experience relevant to the post.
- Mentor training and experience.
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
**What We Offer**
Competitive salary: £32,000 - £40,000 per annum, depending on experience.
