The Complex Disability Team are seeking a skilled Social Worker to join their established team, providing support to individuals with complex health and social care needs. The successful candidate will work closely with colleagues to ensure manageable caseloads and deliver high-quality services.
Key Responsibilities:
- Conduct assessments and develop care plans to meet individual needs
- Work collaboratively with colleagues and external agencies to provide comprehensive support
- Monitor and review care plans to ensure effective outcomes
