Location of post: Neath Civic Centre but also work under the Local Authorities Hybrid Working Policy.

The Complex Disability Team are seeking a skilled Social Worker to join their established team, providing support to individuals with complex health and social care needs. The ideal candidate will have a strong understanding of the Social Services and Wellbeing (Wales) Act 2014 and be able to work effectively with individuals, families, and carers to achieve positive outcomes.

Key Responsibilities:

  • Conduct assessments and develop care plans that meet the needs of individuals with complex disabilities
  • Work collaboratively with other professionals to provide a holistic approach to care
  • Provide emotional support and guidance to individuals, families, and carers
  • Participate in regular supervision and case discussion forums to ensure high-quality practice
