Job Description

We are seeking an experienced Mental Health Practitioner to join our dynamic team in the Liaison Psychiatry service. This is a challenging and rewarding role that will enable you to make a significant difference in the lives of patients and their families.

As a Mental Health Practitioner, you will work closely with colleagues from various disciplines to provide comprehensive care to patients with complex mental health needs. You will be responsible for conducting assessments, developing care plans, and implementing interventions to promote recovery and well-being.

The successful candidate will have a professional mental health qualification, such as an RMN or CQSW, and will have experience working in a community mental health setting. They will be able to demonstrate strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Duties and Responsibilities

  • Conducting comprehensive assessments of patients with complex mental health needs
  • Developing and implementing individualized care plans

  • Working closely with colleagues from various disciplines to provide comprehensive care
  • Communicating effectively with patients, families, and carers
  • Participating in service development and quality improvement initiatives
Requirements

The successful candidate will have:

  • A professional mental health qualification, such as an RMN or CQSW
  • Experience working in a community mental health setting
  • Strong communication and interpersonal skills
  • The ability to work independently and as part of a team
  • Knowledge of evidence-based interventions and care planning
