About the Position:

In this critical role, you will provide support to individuals with autistic spectrum conditions and associated complex needs, enabling them to live as independently as possible. You will work closely with a multidisciplinary team to develop and implement personalized care plans, promoting independence and enhancing quality of life.

Key Responsibilities:

  • Deliver therapeutic care and support to patients with autistic spectrum conditions and associated complex needs
  • Monitor and record patient conditions, treating each individual with dignity and compassion
  • Assist residents in achieving their goals through a care pathway, promoting independence and community integration
  • Collaborate with the care team to ensure seamless service delivery

Requirements:

  • A minimum of 6 months' experience working within a social care setting
  • Hold a current Enhanced DBS on the update service or be willing to obtain one
  • The right to work in the UK
