We are seeking a highly organized and customer-focused Care Navigator to join our team at Gateacre Medical Centre. As a Care Navigator, you will be responsible for providing exceptional patient care and ensuring a smooth experience for our patients.
Key Responsibilities
- Receive and direct patients to the appropriate healthcare professional in a courteous and efficient manner.
- Provide general assistance to the practice team and project a positive image to patients and visitors.
- Manage patient records and maintain accurate and up-to-date information.
- Process prescriptions and other administrative tasks as required.
- Ensure the waiting area is clean and tidy and that all necessary supplies are available.
Requirements
- Experience working in a healthcare setting is an advantage but not essential.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- High level of organization and attention to detail.
