About the Role
We are seeking a highly skilled and organized Care Navigator to join our team at Gateacre Medical Centre. As a Care Navigator, you will play a vital role in ensuring that patients receive the best possible care and support.
Main Responsibilities
- Provide exceptional patient care and support, ensuring that patients receive the best possible experience.
- Work closely with the practice team to ensure seamless communication and coordination of patient care.
- Manage patient records and maintain accurate and up-to-date information.
- Process patient requests and respond to queries in a timely and efficient manner.
- Collaborate with other healthcare professionals to ensure that patients receive comprehensive and coordinated care.
Requirements
- Experience of working in a GP surgery or healthcare setting.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- High level of organization and attention to detail.
- Ability to maintain confidentiality and handle sensitive information.
