About the Role

We are seeking a highly skilled and motivated Mental Health Practitioner to join our Chester Community Mental Health Team.

The successful candidate will work within a multi-disciplinary framework, undertaking assessments and coordinating the care of patients with both functional and organic mental illness.

Key responsibilities will include key-working a caseload, undertaking ongoing assessments, formulating care plans, conducting risk assessments, and providing tailored interventions to improve wellbeing and outcomes for individuals and their families.

Excellent communication, report writing, and assessment skills are essential for this role, as is a comprehensive working knowledge of mental health disorders and the Mental Health Act.

We offer a supportive and friendly team environment, with opportunities for supervision, training, and personal development.

As a Mental Health Practitioner with Cheshire and Wirral Partnership NHS Foundation Trust, you will be part of a team that is committed to delivering high-quality care and services to our local community.
