Job Summary

Main Responsibilities

As a Practice Nurse, your key responsibilities will include:

  • Implementing health promotion and wellbeing programmes to support patients with long-term conditions
  • Developing and implementing individual care plans for patients with chronic diseases
  • Managing and supporting patients at risk of developing long-term conditions, providing advice to promote their health
  • Providing wound care and running specialist clinics such as ear syringing and contraception
  • Collecting specimens for pathology and providing travel advice and vaccinations
  • Requesting pathology as necessary and processing and interpreting limited test results
  • Maintaining accurate contemporaneous clinical records and ensuring data is coded appropriately
  • Implementing an effective call/recall system and immunisation programme
  • Chaperoning patients as necessary and supporting the clinical team in dealing with clinical emergencies
  • Contributing to practice targets and liaising with external services and agencies to ensure vulnerable patients are supported
