Practice Nurse

Thornton Heath Medical Practice is seeking a skilled and dedicated Practice Nurse to join our team. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the overall success of our practice.
Key Responsibilities:- Implement health promotion and wellbeing programmes to support patients with long-term conditions
- Develop and implement individual care plans for patients with chronic diseases
- Manage and support patients at risk of developing long-term conditions, providing advice to promote their health
- Provide wound care and run specialist clinics such as ear syringing and contraception
- Collect specimens for pathology and provide travel advice and vaccinations
- Request pathology as necessary and process and interpret limited test results
- Maintain accurate contemporaneous clinical records in line with current regulations
- Ensure data is coded appropriately and implement an effective call/recall system
- Implement an effective immunisation programme in line with the national schedule
- Chaperone patients as necessary and support the clinical team in dealing with clinical emergencies
- Contribute to practice targets (QOF, local targets, etc.), complying with guidance as necessary
- Liaise with external services and agencies to ensure vulnerable patients are supported (safeguarding)
- Degree in nursing and registered nurse on the nursing and midwifery council register
- Experience of working in general practice as a practice nurse
- Experience of working autonomously and comprehensive understanding of infection prevention and control measures
- Desirable: experience across the range of primary care nursing duties, including long-term conditions, experience of prescribing and undertaking medication reviews, and experience of working in a supervisory or mentoring role
