Job Summary

An exciting opportunity has arisen for a motivated, dedicated, and innovative Physiotherapist to join our multidisciplinary team at Cambridgeshire and Peterborough NHS Foundation Trust. We are a small, dynamic team whose primary aim is to prevent admission to hospital of patients within Huntingdonshire. We work within an acute hospital's Emergency Department (ED) and Acute Assessment Unit (AAU), receiving referrals from consultants requesting assessments of patients suitable for discharge.

Main Duties of the Job

Identify patients within the Emergency Department who may meet the admission criteria for CPFT services. Assess the identified patients or those referred by consultants in the Emergency Department. Document assessments and findings on SystmOne, Etrac, and Symphony. Liaise with consultants and senior nurses in the department. Liaise with the social worker regarding patients who do not meet Intermediate Care admission criteria or who have obvious need for social care support. Support students, learners, and other team members.

Key Responsibilities
  1. Be professionally and legally accountable for all aspects of your own and delegated work.
  2. Work within codes of practice and professional guidelines.
  3. Undertake holistic assessment (including those with complex presentations and multi-pathologies) making use of specialist clinical reasoning skills.
  4. Use evidence-based practice to ensure clinical care is effective and appropriate.
  5. Promote independence and wherever possible avoid hospital admission to ensure those patients receive the appropriate care in the most appropriate setting.
  6. Organise and manage your own time, delegating work appropriately.
  7. Conduct risk assessments and health and safety assessments and immediately report any changes or newly identified risks.
  8. Demonstrate the importance of gaining patient and carer consent in all interventions.
  9. Effectively communicate verbally with patients, carers, and colleagues using tact and persuasive skills.
  10. Actively involved in the development of the service/organisational aims and objectives of CPFT.
  11. Work effectively within the Intermediate Care, Front of House team structure and liaise appropriately with members of the MDT and other agencies by attending MDT meetings, case reviews as required.
  12. Ensure that accurate and timely written records are kept which comply with the Trust policy and HCPC guidance, reporting on any issues as appropriate.
  13. Provide specialist advice to all members of the multi-disciplinary team, service users, and appropriate others, of changes involving care pathways and other relevant matters that pertain to the care of the patient.
