Job Summary:
Horizon Care and Education is seeking a highly motivated and empathetic Young People Support Specialist to join our team in Melksham.

Duties and Responsibilities:
- Developing and implementing personalized support plans that cater to the unique needs of each resident.
- Engaging in regular reviews and updates with residents, family members, and multidisciplinary teams to ensure effective service delivery.
- Participating in ongoing training and professional development to maintain expertise in areas such as trauma-informed care and mental health support.
