We are seeking a dedicated and compassionate Palliative Care Service Leader to oversee our Hospice Community Services team in South West Essex. As a leader, you will be responsible for managing a team of Community Nurses providing palliative care in the community.

About the Role
  • Provide guidance and oversight to the team to ensure high standards of care are delivered to patients and their families/carers with palliative care needs.
  • Support teams in delivering care at the right time to individuals with palliative care requirements.
  • Foster collaborative working relationships with healthcare professionals and agencies to promote holistic assessment and advance care planning.

Key Responsibilities:

  • Manage and motivate a team of Community Nurses to deliver exceptional patient care.
  • Assist in audit and research projects to improve service delivery and quality of care.
  • Monitor and evaluate team performance, providing coaching and support where necessary.

Requirements:

  • A Registered Nurse with post-qualification experience in palliative care or related specialities.
  • Excellent communication and interpersonal skills, as well as a sound knowledge base linked with non-medical prescribing and supervising junior staff.

Salary and Benefits:

£24,473 to £31,034 per annum (pro rata for part-time hours), plus a range of benefits including generous annual leave and professional development opportunities.
