Job Description

Oxford Health NHS Foundation Trust is seeking an experienced clinician to join our Single Point of Access (SPA) team as a Clinical Triage Specialist in Community Health.

We are looking for a highly motivated individual who can work autonomously and within a team, utilising their advanced triaging skills to prevent avoidable hospital admissions.

The successful candidate will be expected to deliver and maintain a high quality of triage, using enhanced assessment, problem-solving, and decision-making skills to identify patient needs and appropriate service destinations.

Key responsibilities include:

  • Referrals to community health services, crisis care, social services, and specialist services such as district nursing, therapy, falls management, and palliative care.
  • Initiating and managing work effectively, with support from the team and training resources.
  • Working closely with clinicians and administrators across two bases on a shift pattern basis.
