As a Social Worker in our team, you will play a pivotal role in supporting individuals with Learning Disabilities, working closely with a multidisciplinary team to ensure that care is delivered in a respectful and dignified manner. Your expertise will be invaluable in helping us achieve our goal of providing person-centred social care that empowers individuals to reach their full potential.
We are seeking a highly motivated and empathetic individual to join our team. If you possess a strong understanding of complex health and care needs, a passion for improving outcomes for adults, and excellent communication skills, we encourage you to apply.
Key Responsibilities:
- Work collaboratively with local people to understand their care and support needs.
- Help identify options available to them and ensure they are connected to the support services and community assets they need.
- Enhance your legal literacy and work closely with our legal support services.
- Develop and progress in your career through training and development opportunities.
- Experience working with people with Learning Disabilities and/or Mental Health needs.
Requirements:
- CQSW/CSS/Diploma/Degree in Social Work or equivalent.
- Social Work England Registration.
- Ability to carry out assessment and support planning in line with relevant legislation.
- Self-motivating individual with a passion to make a difference in adult social care.
