Job Description

Job Title: Reablement Officer

Job Summary:

We are seeking a highly skilled and experienced Reablement Officer to join our team at BlueGreen Recruit. As a Reablement Officer, you will play a vital role in supervising a team of Community Support Workers/Reablement Workers, providing reablement support to service users in their own homes.

Key Responsibilities:

  1. Team Supervision: Supervise a team of Community Support Workers/Reablement Workers, ensuring they are meeting Essential Standards of Quality and Safety and working in accordance with directorate policy.
  2. Service User Support: Provide reablement support to service users in their own homes, working within agreed principles and guidelines with minimal supervision from the line manager.
  3. Training and Development: Manage direct reports and have ongoing responsibility for the development and provision of relevant training.
  4. Quality Assurance: Monitor working practices and quality assurance systems, providing support and guidance to Community Support Workers/Reablement Workers where necessary.
  5. Record Keeping: Maintain records of equipment, monitoring its allocation and ensuring adequate stocks for future use.
  6. Communication: Be accessible for advice and guidance to Community Support Workers/Reablement Workers, providing them with feedback as appropriate.
  7. Performance Management: Undertake formal supervision management and performance development review of staff as required.
  8. Progress Monitoring: Monitor on-going progress of reablement processes and provide support and guidance to Community Support Workers/Reablement Workers.
  9. Liaison: Liaise with commissioning staff as necessary in respect of agreed assessed needs, goals, and outcomes to be achieved by the service user.

Person Specification:

Essential:

  1. 4 GCSE Level C qualifications or equivalent
  2. Diploma in Health & Social Care level 2 or equivalent. Principles of Community Care
  3. Good knowledge of Care Planning process and Essential Standards of Quality and Safety. Good knowledge of Health and Safety issues.
  4. Good knowledge of risk assessment processes Working as a care provider
  5. Previous supervisory/management experience in a similar or related setting Working as part of a team.
  6. Ability to work independently using own initiative. Good written and verbal communication skills.
  7. Supervision skills
  8. Organisational and time management skills IT skills.
  9. Ability to fulfil the travel requirements of the post Commitment to providing a high-quality service
  10. Ability to pass on knowledge in an enabling way.
  11. Ability to support and promote service user independence

Desirable:

  1. NVQ Supervisory Level 3 or equivalent
  2. Diploma in Health & Social Care level 3 or equivalent Knowledge of Community Care Act.
  3. Experience of recruitment and selection.
  4. Implementation of quality assurance techniques Dealing with attendance management issues
  5. Ability to chair meetings Ability to produce reports
