Job Title: Community Health Support Specialist

About the Role

We are seeking an experienced and compassionate Community Health Support Specialist to join our team at NHS. As a vital member of our primary care network, you will play a key role in empowering individuals to take control of their health and wellbeing.

Main Responsibilities
  • You will build trusting relationships with patients, families, and carers, co-producing personalised care plans that address their unique needs.
  • Through your expertise, you will identify and address wider determinants of health, such as housing, employment, and financial challenges.
  • You will focus on holistic, person-centred support, empowering individuals to take charge of their wellbeing.

Key Skills and Qualifications

  • A strong understanding of personalised care models and social determinants of health is essential.
  • You should have knowledge of safeguarding policies for children and vulnerable adults.
  • Proficiency in IT systems, data recording, and report generation is also required.

Benefits

This is a full-time position with a salary range of £25,000 - £35,000 per annum, depending on experience. You will also receive a comprehensive benefits package, including training and development opportunities.
