We are seeking a highly skilled Rehabilitation Programme Coordinator to join our Adult Critical Care team at the Leeds Teaching Hospitals NHS Trust. The ideal candidate will have a strong background in nursing, with experience in rehabilitation and critical care. They will be responsible for coordinating the delivery of rehabilitation services, including assessing patients' needs, planning and implementing interventions, and evaluating outcomes.

This is a unique opportunity to develop your skills and expertise in a fast-paced and dynamic environment. You will be working closely with a multidisciplinary team to deliver high-quality care and improve patient outcomes. Excellent communication and organisational skills are essential for this role, as well as the ability to work independently and as part of a team.

Key Responsibilities:

  • To coordinate the delivery of rehabilitation services, including assessment, planning, implementation, and evaluation of individualised programmes of care.
  • To work closely with a multidisciplinary team to deliver high-quality care and improve patient outcomes.
  • To maintain accurate records and reports, including patient assessments, treatment plans, and outcome evaluations.

Estimated Salary: £50,000 per annum
