About the Role

As a Clinical Associate Case Manager at Bush & Company Rehabilitation, you will form part of our dedicated team of specialist clinicians. You will work closely with clients, their families, and multidisciplinary teams to develop and implement effective rehabilitation plans, focusing on maximizing clients' potential and quality of life.

Key Responsibilities
  • Develop and manage client caseloads, tailoring rehabilitation plans to meet individual needs and circumstances.
  • Conduct thorough assessments in clients' homes, identifying key goals and priorities for rehabilitation.
  • Coordinate with multidisciplinary teams to ensure seamless support and facilitate access to necessary services.
  • Manage administrative tasks, including reporting, communication, and liaison with stakeholders.
Requirements

To succeed as a Clinical Associate Case Manager, you must have:

  • A valid clinical registration with an approved clinical body (e.g., HCPC, NMC, SWE).
  • Proven experience in rehabilitation, preferably in a community setting, with a focus on delivering holistic support.
  • Excellent relationship-building skills, enabling effective collaboration with multiple professionals and clients.
  • A willingness to travel extensively across the UK, Scotland, and Wales, supporting clients in various locations.
