Supporting Families Practitioner

About the Role
The HCRG Care Group is seeking a dedicated and detail-oriented Supporting Families Practitioner to review patient health records and ensure their alignment with the National Supporting Families Outcome Framework.
The role involves validating health data against the framework's criteria to support comprehensive family assessments and ensure accuracy in data reporting and service provision.
Main Responsibilities
- Conduct systematic reviews of patient health records to identify relevant data that aligns with the National Supporting Families Outcome Framework.
- Ensure the accuracy and completeness of health information in relation to the framework's outcomes and indicators.
- Complete reports summarising the alignment of health data with the framework's criteria.
- Work collaboratively with system partners, to integrate relevant data into Patient Health records to accurately support National Supporting Families Outcome Framework.
- Adhere to the service referral protocols. Following processes regarding escalation of cases where the level of need becomes beyond scope.
- To identify support needs, and provide signposting for patients and carers families
- To participate in networking with all other relevant areas to enable a seamless pathway of care - Stay informed about updates to the Supporting Families Outcome Framework and best practices in health data management.
- Engage in ongoing training and professional development opportunities related to family support and health record analysis.
