Job Summary

The HCRG Care Group is seeking a dedicated and detail-oriented Supporting Families Specialist to review patient health records and ensure their alignment with the National Supporting Families Outcome Framework.

This role involves validating health data against the framework's criteria to support comprehensive family assessments and ensure accuracy in data reporting and service provision.

Main Duties of the Job

  • Conduct systematic reviews of patient health records to identify relevant data that aligns with the National Supporting Families Outcome Framework.
  • Evaluate the accuracy and completeness of health information in relation to the framework's outcomes and indicators.
  • Complete reports summarizing the alignment of health data with the framework's criteria.
  • Collaborate with system partners to integrate relevant data into Patient Health Records, ensuring accurate support for the National Supporting Families Outcome Framework.
  • Adhere to service referral protocols and escalate cases as necessary.
  • Provide signposting for patients and carers' families to address identified support needs.
  • Engage in networking activities to enable seamless pathways of care, staying informed about updates to the Supporting Families Outcome Framework and best practices in health data management.
  • Participate in ongoing training and professional development opportunities related to family support and health record analysis.

Salary Information

£25,000 - £30,000 per annum (dependent on experience).

Requirements

  • G.C.S.E. English and Maths, Grade C and above, or equivalent qualifications.
  • Experience within an administrative role or customer service environment.
  • Excellent communication and interpersonal skills, both verbal and written.
  • Able to effectively prioritize and coordinate own work alongside work of others.
  • Standard keyboard skills and ability to use Microsoft Office packages.
  • Flexible to work as part of a proactive team.
  • Excellent time management skills and ability to use initiative without close supervision.
