Job Summary

This is an exciting opportunity to work as a Band 6 Specialist Recovery Practitioner in the Avon & Wiltshire Mental Health Partnership NHS Trust.

\

The successful candidate will be responsible for the care co-ordination of a complex caseload of service users who have psychotic illnesses, including those with co-existing substance misuse issues.

Job Responsibilities

  • \
  • Undertake comprehensive mental health assessments for service users with severe and enduring mental health needs living in the community.\
  • Develop, deliver and review comprehensive treatment programmes using evidence-based practice, including strategies to manage risk for service users with complex needs.\
  • Fulfill duties as a CMHT duty worker on a rolling rota.\
  • Maintain accurate single health and social care records, ensuring both paper and electronic records are up-to-date.\
  • Participate in management, caseload and clinical supervision in accordance with trust policy.\
  • Provide mentoring and training for others in relevant practice areas.\
