About the Role

We are seeking a highly skilled and motivated Mental Health Practitioner/Care Coordinator to join our team at Nottinghamshire Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care and support to individuals with mental health needs.

Key Responsibilities

  • Assess and develop care plans for service users
  • Provide one-to-one interventions and group work
  • Work collaboratively with other healthcare professionals to ensure comprehensive care
  • Conduct risk assessments and develop strategies to mitigate risks
  • Maintain accurate records and reports

Requirements

  • Professional qualification in mental health to degree level or equivalent
  • Current registration with the NMC
  • Practice-based educator qualification or equivalent
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
