Mental Health Practitioner/Care Coordinator Role

About the Role
We are seeking a highly motivated and skilled Mental Health Practitioner/Care Coordinator to join our team at Nottinghamshire Healthcare NHS Foundation Trust. This is a rewarding and vital role within the team, delivering treatment to our patients within a supportive environment.
Key Responsibilities
The successful candidate will be responsible for managing caseloads under the Care Programme Approach, conducting initial assessments and one-to-one interventions with service users, and recording comprehensive need/risk assessments. They will also devise and implement treatment plans, demonstrate the application of specialist skills and knowledge, and establish and maintain robust communication networks with clients, carers, team members, and other healthcare and social care workers.
Requirements
The ideal candidate will have a professional qualification in Mental Health to degree level or equivalent, maintain current registration with the NMC, and have a practice-based educator qualification or equivalent. They will also have good skills in assessment, intervention, and evaluation of service users' care, IT skills, and the ability to prioritise work. Additionally, they will have experience working within a mental health clinical setting and knowledge of current legislation and clinical risk assessments.
