Memory Assessment Nurse

About the Role
This exciting opportunity is available within the Memory Assessment Service (MAS) at Nottinghamshire Healthcare NHS Foundation Trust.
We are seeking a skilled Memory Assessment Nurse to join our team, working with individuals aged 65 and over who have been referred to the service with cognitive impairment.
The successful candidate will work as part of a multi-disciplinary team to screen referrals, complete assessments, and create person-centered care plans.
Main Responsibilities
- Screen referrals for appropriateness and undertake initial assessments
- Create treatment plans and liaise with the multi-disciplinary team to explore symptoms and causes
- Refer for additional assessments and signpost for post-diagnostic support
Requirements
- Must hold RNMH qualification and have substantial experience of MDT working
- Current knowledge and understanding of older people's mental health needs and ability to demonstrate clinical competence
- Substantial and considerable experience in assessing and prescribing for people with dementia
