Job Summary

The Central and North West London NHS Foundation Trust is seeking a skilled and compassionate Complex Outreach Nurse to join our team. As a key member of our multidisciplinary team, you will provide specialist drug and alcohol outreach clinics for local hostels and other services in the London Borough of Hounslow.

Main Responsibilities
  • Provide comprehensive assessments that consider service users' support needs holistically and assess any relevant harm reduction interventions.
  • Develop and maintain pathways into specialist services to ensure the health and well-being of residents is met.
  • Provide health promotion and education to service users, including Hepatitis A & B vaccination and dry blood spot testing.
  • Collaborate with statutory and non-statutory teams, GPs, and primary care services to provide expert care and support.
  • Assess and case manage clients who inject drugs, including assessing injection sites and providing harm minimization advice.
  • Provide clinical advice and support to other colleagues within the healthcare setting and to other agencies concerned with substance misuse.
Requirements
  • Registered Nurse (RMN/RGN) with extensive post-registration experience in mental or general health.
  • Relevant previous post-registration experience of working with drug and alcohol users.
  • Experience in the supply and administration of Hepatitis vaccines and patient group directions.
  • Extensive clinical experience, including case management and risk assessment.
  • Teaching and assessing qualification (ENB 998/SLICE or equivalent).
Desirable Qualifications and Experience
  • Community nursing experience.
  • Experience of working in and delivering services to injecting drug users.
  • Experience of teaching others and willingness to undertake teaching and assessing qualification within six months of commencing post.
Key Skills and Knowledge
  • Sound clinical knowledge and understanding of current NHS policies and national drug issues/policies.
  • Good verbal, written, and presentation skills.
  • Good interpersonal skills to communicate and negotiate effectively, both inside and outside the Trust.
  • Ability to work independently and prioritize effectively.
  • Ability to assess and treat people with dual diagnosis.
