Community Health Nurse

We are seeking a highly skilled and experienced Complex Outreach Nurse to join our team at Central and North West London NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing specialist drug and alcohol outreach clinics for local hostels and other services in the London Borough of Hounslow.
Key Responsibilities- Provide comprehensive assessments that consider service users' support needs holistically and assess any relevant harm reduction interventions.
- Develop and maintain pathways into specialist services to ensure the health and well-being of residents is met.
- Provide health promotion and education to service users, including Hepatitis A & B vaccination and dry blood spot testing.
- Collaborate with statutory and non-statutory teams, GPs, and primary care services to provide expert care and support.
- Assess and manage risk behaviors of clients, provide information and implications of any risks identified on health and social well-being, and work with clients to address and modify risk behaviors.
- Registered Nurse (RMN/RGN) with extensive post-registration experience in mental or general health.
- Relevant previous post-registration experience of working with drug and alcohol users.
- Experience in the supply and administration of Hepatitis vaccines and CNWL Trust Hepatitis Patient Group Directions.
- Ability to work independently and prioritize effectively, with good verbal, written, and presentation skills.
- Ability to assess and treat people with dual diagnosis, with evidence of effective leadership and organizational skills.
