Hospital Discharge Specialist

Ackerman Pierce is seeking a skilled Hospital Discharge Advanced Practitioner to join their team in Somerset. As a key member of the Hospital Discharge team, you will play a vital role in supporting adults in their community to remain independent for as long as possible.
Key Responsibilities:- Deliver high-quality, joined-up social care, health, and wellbeing services to enhance rather than replace existing informal support networks.
- Support people out of hospitals in a timely manner and prevent admissions by utilizing community assets to enable them to return home.
- Give people control of the care and support services they receive, ensuring they are delivered where, when, and by the people they want.
- HCPC Registration
- 1-2 years post-qualifying experience within the UK Social Care industry
- Enhanced DBS/CRB (can be obtained)
- Eligibility to work in the UK
- Your own personal consultant
